The Kindergarten AM/PM Session Preference Form will open on Monday, February 28th at 10:00 a.m.
In order to be eligible, students must have an enrollment application submitted with all the required documents first.
- Incomplete immunization records may be submitted as long as the completed record is turned in within 21 days of the enrollment date.
- For exemptions, the Utah Department of Health Exemption Training Certificate is required.
- Birth certificates can be printed in person at most Utah Health Departments.
Classroom Environment Request Forms
Will be available to fill out in the main office starting Monday, March 28th – Friday, April 15, 2022.
Session/Class Placements
Will be emailed by Thursday, May 26, 2022.
